In Utah, beginning no later than the first IEP in effect when a student is 14, or earlier if appropriate, transition must be addressed. It is required by law that your IEP includes transition goals and services. You must be invited to your IEP meeting to discuss this Transition Plan which will assist you as you move into adult life. This transition plan will update goals and services each year until you graduate.
The Transition Plan you will develop must include long term and annual goals, services and activities to assist you in achieving your desired outcomes. In Utah, you and your family must be informed before your 17th birthday on how your rights will change when you officially become an adult at 18 years old.
